Concierge Program

We are pleased to offer you a new Concierge Program for THREE different types of patients in our practice!

There are THREE levels/THREE patient types:

• PRIMARY CARE
• WELLNESS ONLY
• HORMONE REPLACEMENT THERAPY

If any of these overlap for you, then we can design a program that fits your needs specifically. This is for patients to consider or enroll in after they have established with us as a provider. The initial visit remains at $260.

Level 1: PRIMARY CARE - EXISTING PATIENTS

This program is $120 a month and includes 6 visits a year plus unlimited access by phone Monday through Friday and one call on the weekend once a month. Credit card payments are filed monthly on the 5th of every month. Labs continue to be paid through your insurance company.

Level 2: WELLNESS PATIENTS

This is for patients who see us for nutrition, weight loss, thyriod, lyme, neurotransmitters, PTSD, adjuntive cancer treatment, immune support, or any type of anemia. These patients pay $125/monthly or $1500 a year for the following:

1. Unlimited access by text
2. 15-minute phone calls twice a month
3. FOUR visits a year
4. FOUR IV infusions a year

labs will be paid for through insurance.

Level 3: HORMONE THERAPY PATIENTS

This is for men or women who get BHRT ONLY. For $100 a month you get:

1. UNLIMITED access by text
2. ONE phone call a month
3. FOUR Pellet procedures/Office visits
4. ONE extra visit not related to BHRT for other problems.

Any of these levels will get you a credit of $25 for each referral you make to the practice that comes in and has a visit, which can be used on any service not already offered in the level you are in. An example is an extra IV infusion or test you may want done in the clinic such as: GI Map, ZRT Neurotransmitter Test, or a Covid Test.
